Nationals Catering Order Form

At Nationals, catering is supplied to all participating players at no cost and is included as part of the entry fees. Catering includes morning tea, lunch and afternoon tea.

• Use the form below to indicate your catering preferences for both the Champions Trophy (Saturday) and the National Teams event (Sunday).
• Note we cannot guarantee you will receive your preference - the purpose of this form is to help us order in roughly the right quantities of particular items of food.
• The form should only be filled for players attending one or both of these events.
• Fill in the form for each player.
• Catering preferences must be completed by Tuesday 23 October 2018.
